Lyon-Martin's Surgery Questionnaire

Attached is a questionnaire Lyon-Martin provides to patients to help guide the surgery readiness assessment appointments.  The information gathered is then used to write the letters of medical necessity for surgery, which are then submitted to both the insurance company and the surgeon. This process can also help the patient and their care team to proactively prepare for any anticipated needs before, during, and after surgery.
